How We Work
From developing an In Home Support plan and matching you with a Direct Care Worker, to working with your case manager to develop a care plan, your feedback is essential to providing you or your loved one with the highest quality service.
Initial Call
The initial contact with Soreo from you, your physician, case manager or loved one, is the preliminary stage when we identify you or your loved one’s specific needs. We assess your needs, required schedules, family involvement, and any language requirements (bilingual Direct Care Workers are available upon request).
Continuum of Care
Whether the care you need is required for a limited and specific injury or illness, a special needs child, comfort for end of life care or a combination of care issues, Soreo can provide services throughout all of the stages of your need. Our full continuum of care includes In Home Support Services, Hospice & Palliative Care and Bereavement Services. What this means to you is that there is no need to contract with multiple providers to serve your individual needs.
Availability
All of our services are available at all hours of the day, seven days per week.
Paying For Services
Soreo works with many insurance companies and will work with your case manager to obtain the appropriate approvals for services. If you or your loved one has an insurance policy that does not offer home-based services, we are happy to provide the services you need using our competitively priced private pay option.
Soreo is proud to be Southern Arizona’s largest provider of In Home Support Services to Medicare and Medicaid recipients who have applied and are determined eligible for ALTCS.
